Incident Investigation
Purpose of the course:
This training delivers an essential introduction to the practicalities of incident reporting. It is designed to equip employees with applicable knowledge regarding the various incident and accident investigation tools as well as the importance of understanding the Occupational Health and Safety Act, with regards to incidents and accidents.
Target group:
Management and supervisors. Health and Safety representatives. Anyone responsible for accident investigation reports within their organisation.

SECTION 1: Explain the specified requirements for investigating workplace incidents

⭄ Six steps for successful incident investigation:
⭄ The extent of the inquiry is explained
⭄ Root cause analysis
⭄ Apparent cause analysis
⭄ Incident trend analysis
⭄ Relevant hazards and risks likely to be encountered during the investigation.
⭄ The purpose of conducting investigations into workplace incident prevention
SECTION 2: Prepare to gather data for the investigation.
⭄ The persons, tools, equipment, and materials are verified as fit for purpose and available
⭄ Relevant hazard and risk control measures for workplace incident data gathering
⭄ Consequences of non-compliance
SECTION 3: Gather and evaluate data
⭄ Data is gathered in accordance with the requirements for the intended type of investigation.
⭄ The prevailing conditions at the scene of the incident are determined using accepted data-gathering methods.
⭄ The data gathered is evaluated to establish variances between actual and required standards and procedures.
⭄ The causes of the incident are identified through the interpretation of these variances.
⭄ The importance of identifying the causes of the incident under investigation and the consequences of noncompliance with any required steps is explained.
